UK Pensions ‘Proof of Life’ Many people have received a ‘proof of life’ for from the UK Pension Service. This form requires to be ‘signed off’ by a witness as listed below. You need to present yourself with photo ID to one of the following and sign the form in front of the witness. Bank / Building Society official or accountant Barrister, solicitor or any person allowed to manage oaths in the country where you reside Doctor or dentist. Physiotherapist or pharmacist Government official or local mayor (Muktar) Magistrate, JP Local police officer Care home Manager • My Doctor wanted €10 each, the Muktar wanted €15 each...the Pharmacist did it FREE |

In the past I have used the Funeral Director who will be taking care of that side of things when the time comes! |
